Our Theory of Change reflects a practical pathway from reentry to employment stability, connecting participants, employers, and supportive services to build sustainable careers and stronger communities.

Inputs

Hand2Heart brings together justice-impacted participants, employer partners, workforce coaches, industry-aligned curriculum, and community and funding support to create a strong foundation for workforce success. These inputs ensure participants have both the guidance and the opportunities needed to move forward

Activities

Participants engage in workforce readiness training, individualized reentry employment support, career pathway planning, and employer engagement. At the same time, Hand2Heart works with employers to build understanding and capacity for successful second-chance hiring.

Outputs

Through these activities, participants complete training, earn credentials, secure employment, and connect with employers committed to second-chance opportunities. These measurable results reflect progress for both participants and employer partners.

Short-Term Outcomes
